that would require custom template or form placed in a form module.

Thanks for your reply Klas, I tried a module with search form, it shows the form as I want, 

but one more problem started:

No search results found may be because of the url is not correct like : 

http://bd.rnspublicity.com/index.php/component/cck/?sstate=&sdistrict=&scity=&category_of_item=&company_name=shri&cck=entry&search=free_listing&task=search

when I replace "component/cck/" with the menu title (directory) like the below url manually in the address bar, then it works :

http://bd.rnspublicity.com/index.php/directory?sstate=&sdistrict=&scity=&category_of_item=&company_name=shri&cck=entry&search=free_listing&task=search

Kindly shed some light...

In the search module set target menu item, default Inherited will pick itemid from current page which is probably not correct in your case.
