Form not save data at all

Hi, Seblod Masters
***sorry for my bad english

I created Order Form with Seblod on my site.
You can access it for live here : http://jasaoke.com/order-from.html

I only added 2 fields there : id and product_id.
When I tried to submit some value, nothing was saved to database.

I attached my configs here, so you can evaluate what's wrong.
