I'd like to show a dynamic select drop down to a special user access level. All the other users should get a different field. The difference is a little complicated so I can not solve this by changing the MySQL call. Because the fields are cascading selects I can't do it with CSS either.
Any ideas?

Just make field fore ach group and set Access on the field to desired access level.
I have a lot of groups.... But there is no "every group but group123"...
You should make a viewlevel with all groups but group123 and select it in Access column. This is system limitation - it is how Joomla viewlevels work
My groups are very "lively" - there a new groups every couple of days. So that won't work. Then I have to use BeforeRender to change lists. :-/
Alternative way would be to make a Joomla plugin to change usergroups included in this viewlevel after usergroup is saved/changed.
