Have you checked the already notorious max_input_vars?

P.S. In my case the field stayed in the form and the field was mapped to a (non existing) column my_custom_field.

Viktor did solve the problem, and it was an issue with max_input_vars.

Yeah max_input_vars is the problem.... Amaze how this simple thing slip out my mind when troubleshoot this issue

As I do not have any control over php.ini on my web hotel, I added a line in .htaccess (in root public_html): "php_value max_input_vars 2000".
